    instructions for completion (small business owner use)

    what this sheet is:


    this data for business credit application sheet is your single source of truth for business credit applications. complete every field and use this same sheet each time you apply for vendor accounts, net-terms, business credit cards, equipment financing,
    lines of credit, or any business loan.

    before you start (have these ready):

    • articles/certificate of organization or incorporation (state filing)

    • ein confirmation letter (irs cp 575 or equivalent)

    • d-u-n-s® number (if you have one)

    • registered agent name + address (from state filing)

    • business bank account details (routing + account number)

    • naics code (6-digit)

    • last 12 months gross revenue (or a reasonable projection if new)

    • 3 trade references (vendors) with account details and contact info

    rules (must follow):

    • no blanks. if something doesn’t apply, write “n/a” or “none.”

    • match your legal records exactly across state, irs, and bank records.

    • use business contact info only (business email and phone).

    • use consistent formatting every time.

    • accuracy beats speed. mismatches can cause denials.

    Section 1: Header

    (NAME OF BUSINESS)

    • Enter your legal business name exactly as registered (include “LLC,” “Inc.,” etc., if applicable).

    Section 2: Business Contact Information

    name & title

    • enter the person completing the form and/or authorized signer (example: sandy day, managing member).

    • use a title that matches your structure (owner, president, managing member, partner).

    company name

    • enter the legal business name (same as the header).

    ftin

    • enter your ein (federal tax id).

    • if you are a sole proprietor using ssn (not recommended for business credit), enter ssn only if the application requires it.

    duns

    • enter your d-u-n-s number if you have one.

    • if you don’t have one, write “n/a” (or “pending” only if you’ve already applied).

    phone / fax / company e-mail

    • use your business phone and business email (ideally tied to your domain).

    • if you have no fax number, write “n/a.”

    registered company address

    • enter the address tied to your state registration and used consistently across your business records.

    • use a deliverable address (if you use a suite, include it).

    • do not use mailbox locations (ups store / mailbox express) or p.o. boxes.

    city / state / zip code

    • match the registered address exactly.

    date business commenced

    • use your official start date (often the state formation date or the date you began operations).

    • be consistent.

    legal business structure

    • select or enter one: llc, corporation, s-corp, c-corp, partnership, sole proprietorship, nonprofit, etc.

    • if available, include the formation state (example: llc (va)).

    dba

    • if you operate under a “doing business as” name, enter it exactly.

    • if none, write “none.”

    section 3: business and credit information

    Registered Agent / Address / City / State / ZIP

    • Enter the Registered Agent name and address exactly as shown on your state filing.

    Purpose of Business

    • Write a clear, simple description of what you sell/do (1–2 lines).

    • Example: “Commercial cleaning services for offices and retail locations.”

    NAICS

    • Enter your 6-digit NAICS code that best matches your primary revenue activity.

    Bank Name / Bank Address / Phone / City / State / ZIP

    • Enter the details for your business bank account.

    • Use what’s on your bank statements or online banking profile.

    Type of Account / Account Number / Routing Number

    • Enter Checking or Savings (or both if applicable).

    • Enter the routing number and account number carefully (double-check digits).

    • If you have both a checking and savings account, list the primary account used for business transactions unless the lender requests otherwise.

    Annual Revenue

    • Enter gross annual revenue (not profit).

    • Best practice:

    • Established businesses: last 12 months gross revenue.

    • New businesses: conservative projected annual revenue (be realistic and consistent).
